Nobody makes a good decision about surgery while holding a clipboard in a waiting room, five minutes before it starts. So every form we use is here, in full, at a permanent link you can open on your phone at your kitchen table — read it twice, look up a word, write down the questions you want answered before you agree to anything.

Each one walks the same path: what's being proposed, what it should do for you, the risks that genuinely matter, the alternatives including doing nothing at all, and what we need from you afterward. What we will never do is treat a signature as the end of the conversation. Your treating dentist still owes you the discussion — the version specific to your mouth, your health history, and your scan. Bring every unanswered question to it.
