Before you sign

Why this medication history matters

Bisphosphonates, denosumab, and certain antiangiogenic or cancer medicines can alter bone turnover and healing. Medication-related osteonecrosis of the jaw (MRONJ) is uncommon but potentially serious, especially after extractions, implant surgery, grafting, or in patients receiving higher-dose cancer therapy.

The treating dentist may need to coordinate with the prescribing clinician. Medication changes or a “drug holiday” must never be started without the prescribing clinician’s direction.

Material risks and possible complications

  • Delayed healing, exposed or non-healing bone, pain, infection, drainage, swelling, numbness, loose teeth or implants, fracture, or need for prolonged treatment or surgery.
  • The condition can occur without dental surgery and can persist despite appropriate care.
  • Stopping or delaying the medication may carry separate medical risks, including fracture or progression of the condition being treated.

Reasonable alternatives

  • Non-surgical or tooth-preserving treatment when appropriate, deferral, specialist referral, modified surgical technique, or no treatment.
  • No dental treatment can allow pain, infection, or disease to continue or worsen.
