Proposed treatment

Peri-implantitis is inflammation around an implant that has already caused bone loss. Non-surgical treatment cleans the implant surface and the pocket around it without lifting the gum — using hand and ultrasonic instruments, air-polishing powders, irrigation, and sometimes a local or systemic antibiotic.

Material risks and limitations

The infection may not resolve. Non-surgical treatment reaches a limited depth, and an implant surface is far harder to clean than a tooth root.
The gum commonly shrinks back as the swelling settles, so more of the implant or its metal collar may show afterward.
Sensitivity, soreness, bleeding, and temporary looseness of the gum cuff.
Bone already lost does not grow back with this treatment. The goal is to stop further loss, not to rebuild.
Surgery, or removing the implant, may still be needed. Repeat treatment and long-term maintenance visits are usual.
Antibiotics, where used, carry their own risks including allergy, stomach upset, and resistance.

Reasonable alternatives

Surgical peri-implantitis treatment, implant removal with or without site reconstruction, continued maintenance and monitoring alone, or no treatment.

If nothing is done

Bone loss around the implant usually continues, and the implant can loosen, become painful, or be lost. Bone lost first makes any future replacement implant harder to place.
