Before you sign

Review this document before your appointment when possible. Your treating dentist must still explain the patient-specific treatment, benefits, material risks, alternatives—including no treatment—and answer your questions. Do not sign after receiving sedating medication.

2. What you are agreeing to

Proposed treatment

The planned tooth will be removed and, if the site is suitable at surgery, a dental implant may be placed during the same visit. Bone-grafting material, a membrane, a healing component, or a temporary tooth may also be used.

Immediate placement is conditional. Infection, bone defects, inadequate stability, unexpected anatomy, or other findings may require grafting and delayed implant placement instead.

Expected benefits

  • Fewer surgical visits and a shorter overall treatment sequence when the site is suitable.
  • Preservation of the extraction-site contours and earlier progression toward a replacement tooth.

Material risks and possible complications

  • All risks of tooth extraction and dental implant placement.
  • Failure to obtain adequate initial stability, requiring the implant plan to be changed or abandoned.
  • Infection, bone or gum recession, soft-tissue contour changes, graft loss, implant exposure, or implant failure.
  • The temporary restoration may need to be shortened, repaired, removed, or kept out of contact during healing.

Reasonable alternatives

  • Remove the tooth, allow healing, and place an implant later.
  • Socket preservation or other grafting followed by delayed implant placement.
  • A bridge, removable prosthesis, or no replacement.

What happens to what you type

While you type, nothing is sent and no draft, analytics, or advertising script records your answers. Download builds the PDF in your browser, saves it to your device, and sends nothing — this website never transmits your completed form. Hand it to our staff at your visit so they can place it in your record.
