Before you sign

Review this document before your appointment when possible. Your treating dentist must still explain the patient-specific treatment, benefits, material risks, alternatives—including no treatment—and answer your questions. Do not sign after receiving sedating medication.

Proposed treatment

A custom guard is made from impressions or a scan and worn — usually at night — to spread the force of clenching and grinding. For implant patients it protects the implants, the screws, and the zirconia or acrylic teeth from forces that are among the commonest causes of chipping, screw loosening, and fracture.

Material risks and limitations

  • A guard manages the forces of grinding. It does not cure grinding or clenching, and it does not treat sleep apnea.
  • Temporary soreness of the teeth or jaw, increased saliva, dry mouth, or altered speech while adapting.
  • Teeth can shift if the guard is worn inconsistently or is not adjusted when it stops fitting.
  • Changes to the bite, jaw joint symptoms, or jaw muscle soreness; tell us promptly if these occur.
  • The guard wears through, cracks, or breaks and is a replacement cost. Heavy grinders may need a new one every few years or sooner.
  • Cleaning it daily matters: a guard that is not cleaned harbors bacteria and yeast and can cause gum inflammation or a fungal infection.
  • Never wear a guard that has become loose, sharp, or damaged.

Reasonable alternatives

  • No guard and accepting the risk of wear, chipping, or fracture; a different guard design or material; occlusal adjustment; a stock over-the-counter guard, which is not custom-fitted and can move teeth; treatment of an underlying jaw joint problem; or medical evaluation where sleep-disordered breathing is suspected.
