Your name and number come last, on the final screen, and nothing at all is transmitted until you press the button there.

A patient coordinator calls you back — a real person from this office, not a call center — to answer questions and, if you want one, book a free consultation. There is no obligation attached to finishing the quiz, and no salesperson at the other end of it.
