So here are the license numbers, the NPI records, and the registries, with the steps to look up every one.
Both owner-surgeons are Masters of the ICOI, the world's largest implant organization — a credential earned on documented surgical case volume. Dr. Sam Jain holds a DMD and an MS in mechanical engineering from the University of Connecticut. Dr. Arpana Gupta holds a BDS with Honors and an MDS in endodontics. One detail worth sitting with: the doctor who examines you is the doctor who treats you, and the doctor who restores you. No fly-in surgeon places your implants and disappears. When you look these two up in the registries below, you are looking up the actual hands on your case — not a name on a brochure and a stranger in the operatory.

The most important check takes about a minute. Go to the Dental Board of California license lookup, enter license number 50472 (Dr. Jain) or 50467 (Dr. Gupta), and read the record. It shows license status, the issue date, and the full regulatory history the Board holds — the state publishes all of it, and we would rather you read that record yourself than take our word for any of it. Note that Dr. Jain is registered under his legal name, Sambhav Jain. An active license you have checked yourself is worth more than any claim on a website — including this one. The same lookup also verifies their state sedation credentials: moderate sedation permit MS 493 (Dr. Jain) and MS 376 (Dr. Gupta).
Next, the federal record. Every legitimate healthcare provider has an NPI — a National Provider Identifier — in the public NPPES registry. Dr. Jain's opens directly at npiregistry.cms.hhs.gov, and Dr. Gupta's at the same registry. These are the federal identity records insurers use — they show the name, taxonomy, and practice address on file. They do not, on their own, prove licensure: CMS issues an NPI without verifying it. The Dental Board lookup above is the one that proves a license, which is why it comes first.
Finally, the implant credential. Both doctors are Masters of the International Congress of Oral Implantology. Search the ICOI member directory for "Jain" or "Gupta" with the city of Fremont. How do I verify a dentist's license in California?
Use the Dental Board of California license lookup at search.dca.ca.gov and enter the license number. For us, that's #50472 for Dr. Sam Jain (registered under the legal name Sambhav Jain) and #50467 for Dr. Arpana Gupta. The record shows whether the license is active and lists any disciplinary history. If the link opens a search page rather than a profile, type the license number in and it will pull the record. The whole check takes about a minute and costs nothing. One detail that may catch your eye: our corporation's registered name from 2008 reads "Arpana Gupta, DDS and Sambhav Jain, DDS, Inc." while Dr. Jain's degree is a DMD. DDS and DMD are the same dental degree — some universities simply name it one way and some the other — and the license record, not the corporate filing, is what verifies the person.
Is this dentist board certified, and how do I confirm it?
Both owner-surgeons are Masters of the International Congress of Oral Implantology (ICOI), the world's largest implant organization — a credential awarded on documented surgical case volume, above ICOI Fellowship and below its Diplomate level — which you can confirm in the ICOI member directory at icoi.org by searching "Jain" or "Gupta" with the city of Fremont. What is an NPI, and where do I look it up?
An NPI is a National Provider Identifier — the number the federal government assigns to a healthcare provider so claims and records can be matched to them, and the same one insurers use. It lives in the public NPPES registry at npiregistry.cms.hhs.gov.
