不一定——而一家诊所若在看过 3D 扫描之前就说「都得拔」，那是在背诵一条政策，而不是在下诊断。一颗完全萌出、刷得干净、又能与对颌牙正常咬合的智齿，往往可以保留一辈子。真正该拔的是那些惹麻烦的：顶着邻牙牙根的阻生齿、只萌出一半、在牙龈瓣下藏匿感染的智齿，以及牙刷根本够不到、已经龋坏的智齿。您的免费 CBCT 扫描会把这一切精确地显示出来——包括哪些该拔，以及同样诚实地告诉您，哪些该留着别动。
怎样才算「外科」拔牙？我的需要这样做吗？
简单拔牙拔的是完全露在牙龈之上、看得见的牙。外科拔牙则处理其余所有情况——还埋在牙龈下的、朝邻牙侧向倾斜的、或与骨紧密融合的牙——做法是切开牙龈，有时把牙分割成块，再分次可控地取出。下颌智齿紧邻下颌神经，上颌智齿贴近上颌窦，这正是我们要在 3D CBCT 上、而非在一张平面 X 光片上做规划的原因：扫描会在三维空间中显示神经与上颌窦的位置，于是手术路径在拿起第一件器械之前就已定好，而不是在操作中途才被发现。
